Posting Requirement for OSHA 300A

The new OSHA Form 300A, which is a summary of each business' illness and injury reports for 2004, must be posted February 1, 2005.

The new form must remain posted for three months, or until April 30, 2005. The posting requirement applies only to the summary form, not to the OSHA 300 log.

The summary must contain the total number of job related illnesses and injuries that occurred in 2004 at each facility or establishment.  Even if your business had zero for its illness or injury experience, you are still required to post the OSHA 300A.

The form must be signed or certified by top facility executive (not by the Safety Manager or Human Resources Manager).

For more detailed information on filling out the OSHA forms, you should see the OSHA web site for a 12 page booklet that explains how to complete the forms at http://www.osha.gov/recordkeeping/new-osha300form1-1-04.pdf
